Acknowledgment of Risk, Waiver, and Disclaimer for the MindOzone Program
IMPORTANT: THIS IS A LEGALLY BINDING DOCUMENT. PLEASE READ THE ENTIRE TEXT CAREFULLY BEFORE SIGNING.
This Agreement is entered into between MindOzone (“Program”) and the undersigned participant (“Customer”).
SECTION 1: DISCLAIMER – PROGRAM IS NOT MEDICAL OR THERAPEUTIC ADVICE
The Customer expressly acknowledges and agrees that Fata (Leila) Pezeshk and the MindOzone Program are NOT providing medical advice, psychological counseling, or therapeutic services.
Leila Pezeshk is not a licensed therapist, psychologist, or certified coach. All information, materials, and recommendations offered through MindOzone are based on her professional background, education, independent research, and practical experience in neuroscience, organizational psychology, and behavioral change.
The Program’s content is provided for educational and informational purposes only and is not a substitute for professional medical or mental-health advice, diagnosis, or treatment from a licensed healthcare provider. Participants should always seek the advice of qualified professionals regarding any medical, psychological, or health-related condition.
SECTION 2: CUSTOMER ACKNOWLEDGMENTS
By signing this document, the Customer confirms that they have read, understood, and voluntarily agree to each of the following points:

Consultation with Healthcare Provider
I understand that I am responsible for consulting with my own physician or qualified healthcare provider to ensure that the practices suggested in the Program are suitable for my individual health needs and conditions. -
Personal Responsibility & Assumption of Risk
I acknowledge that I am solely responsible for my own health, decisions, and well-being. I voluntarily assume any and all risks—known or unknown—associated with my participation in the Program. -
No Guaranteed Outcomes
I understand that the Program makes no guarantees or warranties regarding any specific personal, professional, or health-related outcomes or results. -
Release of Liability (Waiver)
I hereby release, discharge, and agree to hold harmless MindOzone, Fata (Leila) Pezeshk, and their respective owners, employees, contractors, and agents from any and all claims, demands, or causes of action arising from my participation in the Program, to the fullest extent permitted by law in the State of California.
